Report a side effect
Drug safety (pharmacovigilance) is of paramount importance to Coripharma and something we take very seriously. If you would like to report an adverse event (side effect) with a product produced by Coripharma, or you would like to report a Product Quality Complaint, please make sure to keep your report as detailed as possible.
What information should I include when submitting a report?
Please report all safety information even if the details are incomplete:
- Name of the product, including dose, strength, frequency and lot number
- A description of the product complaint or side effect associated with the product, including all symptoms
- Your name and contact information (optional)
- Information about the person who experienced the side effect (e.g., initials, date of birth, gender)
Any information you can provide is invaluable for continued patient and drug safety.
